Hey guys,
I'm putting together my documents for my Display Supervisor's but I'm caught on the vagueness of the "work Journal" section. Does anyone have a template they could share or some advice?
Otherwise I was just going to make a spreadsheet.

That's what im using is a spreadsheet with Dates, location, shell sizes,

Dates / Location / shell size / position
Date - self explanatory Location - address of location of shoot Shell Size - 1.3 and or 1.4 / Cakes / BARRAGES / etc... Position - Display Assistant performed Setup / firing / clean up and next day inspection of fire site.
keep everything on spreadsheet and try to keep it up-to-date.

also add in safety taken such as fire extinguishers fire department and such

For the Work Journal requirement on the upgrade form copies of 3 AHJ forms for shows that you worked on as a Display Assistant fulfill that requirement. The Display Supervisor writing the letter (or letters if there is more than 1 that you worked with) should list the show location and date corresponding to the AHJ form and say that you worked with them on those shows (noting both the number of the Display Supervisor and your Display Assistant number). The Display Supervisor should comment on the type of devices used (eg. Display Shells and Cakes) and they should recommend that you are competent to be upgraded to a Display Supervisor or what ever endorsement if the upgrade is for endorsements.
Any other record keeping is good and should not be discouraged but the above is what my company submits to the ERD and it has always (and as recently as a week ago) been accepted by the ERD.
As per the Explosives Regulations Display Supervisors are required to keep records of the shows that you do including Site Plans, Storage locations, AHJ forms. see section 432 of the Explosives regulations below.
432 (1) A fireworks display plan must be prepared in writing and kept for two years after the date of the display. The plan must include the following information:
(a) the name of the display supervisor in charge and the number and expiry date of their fireworks operator certificate;
(b) the location of any Footnote *storage units in which the fireworks to be displayed will be stored before or after the display;
(c) a description of the display site, including the distance in metres from the firing site to the nearest spectators, buildings, structures and Footnote *vulnerable sites;
(d) the type and Footnote *product name of each firework to be used and the name of the person who obtained its authorization;
(e) the quantity of fireworks to be used under each Footnote *product name;
(f) a description of how the fireworks will be positioned within the firing site and how they will be fired;
There is an upcoming amendment to this part of the regulations that says
(g) a description of the crowd-control measures that will be taken; and
(h) an assessment of the likelihood of harm to people and property resulting from the use of the fireworks.
There is an upcoming amendment to this part of the regulations below
40 Paragraphs 432(1)(d) and (e) of the Regulations are replaced by the following:
(d) the type of fireworks to be used;
(d.1) the diameter in millimetres of the largest aerial shell to be used or, if no aerial shells are to be used, the maximum height the fireworks will reach during the display;
(e) the quantity of fireworks to be used

I'll contact my supers and ask them for AHJ forms / show logs and in conjunction I'll create a spreadsheet and submit both; two birds with one stone as it were.
That should go without a hitch eh?
